 Do you have a dream? Tell us about it.

Each year we award a "Dream Come True" to one amazing person with hydrocephalus. All you have to do is send submit the form below telling us why we should present this person with the Hydro Angel Award™. We will honor our chosen Hydro Angel at the annual Hydrocephalus Conference. Make plans to attend this event, this will be a day to remember! The winner MUST be able to travel to Austin,Texas. Once you arrive, food, lodging and fun are on us! Oh... and of course your dream come true award pacakge is too!

Nominate your Hydro Angel on or before December 31, 2014 to be considered for the 2015 Awards.

This event includes VIP cocktail hour, dinner, auction, entertainment, award ceremony, prizes & surprises and laughs!
